Fill in a form the company sent you

Fill in an online form a company sent you or put on its website, add photos or files, and submit it, including what the required and recommended fields mean.

A company can send you a link to one of its forms, or put the form on its own website. The link opens the form with the company's name and the form's title at the top, at an address that starts with /intake/p/. You do not need a portal account.

Steps

1. Read the title and the note under it: they say what the form is for.
2. Answer the questions. A field marked as required has to be filled in; the page says so under any you missed.
3. For an address, start typing in Search for the address and pick the match. If the address is not found, select Enter address manually and fill in the fields.
4. To add photos or documents, drag them onto Drag files here to attach or select Add files. Each file is uploaded and checked for safety; wait until every file shows Ready.
5. Select Submit form.

What you see

• The page shows Submission received with the company's thank-you message. The company now has your answers and contacts you from there.
• Some companies also email you “We received your request” with a short summary, or text you the same. There is nothing to do with it; keep it in case you contact the company.
• Some forms send you on to another page after you submit; the page says Redirecting you to the next step… while it does.
• Beside the title, two small cards describe the form's version and how it is protected. You do not need them to fill in the form.

If something looks different

• Recommended fields are empty: some fields are not required but help the company. Fill them in, or switch on the note that you want to submit without them, then select Submit form again.
• The button reads Waiting for uploads...: a file is still uploading or being checked. Submitting unlocks on its own when it is done.
• A file shows Blocked or Scan failed: it did not pass the safety check. Remove it and add a different file.
• Please review the highlighted fields.: a required field is empty or an answer is not in the expected format. Fix the fields marked in red.
• Form unavailable: the company has taken the form down or the link is wrong. Contact the company.
